Our care managers partner with attorneys to assist with monitoring medical care, watching for safety issues, and assisting with decision making that maintains the best quality care for the aging person. Our extensive experience working with local attorneys in the care of aging persons assures our expertise in the process and our understanding of the local legal processes for guardianship. Guardians
When an individual is unable to care for their own needs or make decisions for their proper care, a court will appoint a guardian to make decisions that benefit the person. Guardians may be a family member or a friend. In many cases, the appointed guardian is an attorney. While the guardian makes the decisions for care, they often need professional assistance in determining the appropriate medical and psychological care. This is where ALCC comes in. As experts in eldercare, a care manager works with the guardian to provide the highest quality of care possible. Care management helps determine current and future needs, as well as making sure that proper care is being provided at all times. If you are a family member who is acting as a guardian of your aging parent or grandparent, or an attorney acting as a guardian, please reach out to us to assist with care management.
Trust Officers
When a care manager works with a trust officer they provide long-term care expertise. They are able to look at the special needs of the person to determine the care required now and into the future. By assessing the client and providing our expert opinion, the trust officer can be certain that all care needs are being met.
A care manager will assess the client’s physical, mental, and social health. The assessment leads to a care plan which outlines both current and future care needs. We write annual reports for special needs and work with the client and trust officer to make sure that the care is managed accordingly. With a care manager involved, services are not duplicated or overlapped, which means that costs are controlled. Our team provides expert guidance related to chronic disease management. Cognitive and memory loss care are special areas of expertise we offer. When a crisis occurs, you can count on us to be involved. Extensive experience with benefits, insurance, the Medicaid Waiver process, and other key resources comes with our involvement.
Ongoing wellness and safety checks are also included in our work and referring to community service providers who operate with integrity is very important. We never receive referral fees or kickbacks for any of our recommendations!

Physicians
Physicians provide frontline medical care for aging seniors. When a family sees a physician, they receive a diagnosis and a recommended plan for continued care. Often, it is difficult for older persons to follow the plan of care when they return home. If they have diminished cognitive capabilities or are struggling with mobility it diminishes their ability to continue proper care. Families are often worried about them, especially if they live at a distance. The physician may want to make a recommendation to follow up with a care manager who can assist with aftercare and make sure that the elderly person is well cared for and the chances of continuing medical issues are reduced.
